Most of the ones I'd recommended have already been mentioned but to  
those I'd add:

Cardew, Cornelius et al,  Cornelius Cardew (1936-1981) A Reader  
Copula, 2006 (ISBN 0-9525492-3-9)

This includes Cardews writings on a number of subjects including on  
his own scores. There is a very interesting essay on Notation as well  
as on the use of same by Stockhausen and others. Certainly essential  
reading for information on his own scores.
